Illuminate does not participate with any insurance carrier. We cannot submit medical claims on your behalf, nor assist you with claim resolution. You are responsible for submitting any claims to your insurance company.
All services are strictly on a self-pay basis; however, we will provide you with an insurance billing invoice (upon request) that you can submit to your insurance carrier for possible reimbursement.
Please keep in mind that there may be procedures and laboratory tests that are non-covered due to your individual insurance policy or plan. Should you have any questions regarding your medical coverage, please contact your insurance carrier. Their telephone number should be on the back of your insurance card.
Also, our providers are not contracted with Medicare; therefore, your consultation and some laboratory charges are not reimbursable by Medicare.

Our cash pay model supports this commitment, enabling us to craft care plans uniquely suited to you, free from insurance constraints. Your well-being is our utmost concern, and this structure lets us serve you best.
This aligns with our 3 business days cancellation policy. It ensures that your appointment time is reserved for you to prevent last-minute cancellations, as we are typically unable to fill your spot on shorter notice due to the structure of our practice.

When you have decided to partner with us, click on the “Contact Us” link and fill out the form. Our staff will then reach out within 2 business days with additional instructions, including how to create a patient portal account (including uploading a credit card for secure billing), which will allow you to see our providers’ schedules to choose an appointment that works best for you.
New patient paperwork should all be completed at least 48 hours prior to your first visit. You will find the new patient paperwork in the patient portal under “Forms and Questionnaires.”
Yes! Please upload previous bloodwork (if done within the past 12 months), consultations with other providers, test results, or any other pertinent information that you would like to share by going to your patient portal, clicking “Labs and Documents” and then selecting “Upload New Document.”
Within 2 business days after your visit, you will find visit summary notes in your patient portal by clicking “My Visit Summaries.” Please read these notes in their entirety as your provider has put together your tailored plan and this will serve as your guide with your next steps.
In your patient portal, click on “Labs and Documents” and then select “Sent/Ordered” to find your lab order/requisition. Although these are usually transmitted electronically to your respective lab, we ask that patients print this out and take it with them to avoid unnecessary delays and errors if the lab’s electronic system is down when you arrive.
Your provider will make recommendations based on your specific health plan and goals.

In our functional medicine practice, we recognize every patient's distinct needs and stories, understanding that healing times vary. We do remind patients to be “patient” (no pun intended) with their healing journey. Because chronic illness develops over time, the path back to wellness also takes time.
Brief questions for clarification about your plan after a recent visit with your provider (1-2 sentences) or administrative questions (billing, scheduling, form requests, etc) may be addressed through the portal. Questions regarding new symptoms or problems (or if you haven’t had a follow up appointment in over 3 months) will require a scheduled visit so that we may spend time appropriately and thoroughly addressing your concerns. The provider will use their best clinical judgment to determine whether your question can be answered via the portal, a 15-minute phone call, or a 30-minute appointment depending upon which would best serve you. The standard rates will apply for phone consultations and follow-up appointments.
Patient packages were created based upon our years of experience in treating patients with a functional and lifestyle medicine approach; this reflects the recommended visits needed to achieve optimal results. To accommodate the varying needs of patients, we have created different levels of packages to align with patients' needs and desires for treatment. The appointments in each package contain our recommended approach for treatment based upon the provider’s clinical judgment after the initial consultation is done (i.e., more complex patients with greater clinical imbalances will often require more visits in their healing journey).
The cost for the initial consultation must be paid in full. Once you choose a package, you may pay in full or split the cost into two payments if desired (this will incur an additional $25 service fee for each of the two payments).
Depending upon your specific health needs, you and your provider will discuss how often you need to be seen. For some patients, visits included in the package may last up to six months, while others may be able to spread visits out over a year (everyone’s healing journey is unique and requires a different number of visits with your provider). Each patient will need to complete the visits in your package within 12 months.
We encourage patients to discuss lab testing with their provider at their initial consultation. Each patient is provided with a customized lab order specific to their diagnoses and long-term desires for health and wellness. For patients with insurance that covers conventional labs, billing and insurance claims for your lab orders are handled directly by the lab facility (Labcorp, Quest, or others). For patients who are self-pay for labs, we will attempt to utilize the most cost-effective option. Overall lab costs may vary based on your provider’s recommendations. Your fee for laboratory tests ordered during the first appointment can range and will depend on the complexity of the condition. Functional specialty labs can range from $100 to $2500, and we will work within your budget to help you prioritize testing. Typically, the rate for the lab fees are lower for follow up appointments since most of the labs are done during the first appointment.
Our providers will create a personalized lab order for each patient during the initial visit. Upon booking a follow up visit with a provider, the lab order will be accessible to the patient via the patient portal. Submitted lab orders must be reviewed by the prescribing provider, and results will be discussed at the follow up appointment.
